GymMaster allows you to set up digital paperwork for members to sign as part of their memberships or bookings. This is managed in Settings > Forms and Waivers.
This area is most commonly used alongside Membership Types, ensuring important agreements, waivers, or questionnaires are completed during sign-up.
Available Form Types
At the top right of the Forms and Waivers page, you will find three buttons :
+ Agreement
Commonly used to create Terms and Conditions or Waivers for members to sign.
Ensures legal and policy acknowledgement before a member begins using the facility.
+ Questionnaire
Allows you to set up a Questionnaire for members to complete.
Useful for collecting additional information during sign-up.
Can also be attached to specific booking types (e.g. a health screening form before attending a fitness class).
+ Contract Summary
Creates the Membership Contract Summary, presented during the sign-up process.
Summarises the membership details for the member.
Ensures members understand what they are signing up for.
